The Medical Student Performance Record (MSPR), previously referred to as Dean鈥檚 Letter, can be sent to third parties for fellowship and/or licensure applications at the request of 平特五不中 MD, CM program graduates. Such requests are sent by encrypted e-mail directly to the program, without exception, and are subject to the fees shown below. Please note that MSPRs are not updated for credentialing purposes; it is the MSPR last updated for R-1 matching purposes that will be sent.

Requests should be completed via the online . It is your responsibility to ensure that all addresses are correct. Addresses for each program requesting your MSPR must be entered simultaneously on the initial online application, by clicking on "Next Program," and filling out the form(s). MSPRs will only be sent to institutional e-mails; personal e-mails are not accepted. Once all program requests have been entered, click on "Final Program."
Also, the 平特五不中 Authorization For Release of Personal Information form must be signed and uploaded to the E-store.
Each request is treated, processed and charged separately. Once the request and payment have been received, you will be able to track the status of your MSPR Request by using the 鈥渧iew request status鈥� link provided in the confirmation email you will receive.
Please allow a four-week processing period starting from the date both the online payment and required documents have been received.
