Supporting Documents

Applicants are required to use the document upload feature during submission of their applicaiton.

Do NOT mail or fax a copy of the documents that you have already uploaded to your application—doing so will slow down the review of your application.

1. Official Transcripts of all previous university education

Applicants must upload a copy of their unofficial transcript during application submission.

Our Admissions Office reserves the right to require official academic records at any time during the admissions process, and rescind any offer of admission made if discrepancies between unofficial and official record(s) are found. Official transcripts sent to ƽÌØÎå²»ÖÐ become the property of the University and will not be returned or forwarded to other institutions.

2. A Minimum of Three Reference Reports or Letters

  • One from the Dean of your Dental School
  • One from an Oral and Maxillofacial Surgeon whom you have worked with or the Residency Program Director at your school
  • One from a dental professional who is familiar with your academic ability.

When you are completing your application, you will be asked to submit your referees’ emails; referees will then be contacted directly via an automated email to upload their letter.

Please note that all documents submitted to ƽÌØÎå²»ÖÐ in support of an application to be admitted, including, but not limited to transcripts, diplomas, letters of reference and test scores (in paper or electronic format), become the property of ƽÌØÎå²»ÖÐ and will not be returned to the applicant or forwarded to another institution.
