This qualitative research examined the complex, multi-level influences and meanings of transition within specific care communities involved in the transition to adult care, analyzed through the framework of symbolic and social boundaries theory. The findings highlight cultural clashes inherent in the disparate care system. Healthcare professionals often engage in informal practices, fostering interdependence through coordinated health corridors that establish practice structures.
